Shackles: four 25T shackles, one's broken while lifting the block
Description of the accident:
When the block was lifted about one meter high by four 25T shackles,
One of the shackles was suddenly broken, then the loose side of block landed and hit the ground heavily.
One corner of the bulkhead plate was damaged, but fortunately no casualty or injury happened.
Solutions always come after the accident. The yard finally sent all the serving shackles to NDE department and three 35T shackles and two 25T shackles were found with cracks.
When we are working in the yard, it is vital to keep the S.T.O.P. in mind and DON’T stay under any objects which have been lifting in the air! (Be careful when searching shadow in summer
